This video of one of my lifters perfectly demonstrates how a slight change in balance can affect your lifts. If you observe the first pull, his hips get just a little higher faster than his shoulders. Although he can still scoop into the power position, the entire combined mass of the lifter and barbell are very slightly overbalanced forward. The pulling dynamic is excellent, but once the bar comes into the catch position, the body is slightly off balance forward and the weight is lost in front.
