										<content:encoded><![CDATA[<p><strong>No one ever expects a death to occur during physical training or an athletic contest.</strong> When it does occur, not only is it distressful to family members, teammates, and coaches, it leaves everyone scratching their heads, thinking “why?” and “could this have been prevented?”</p>
<p><strong>In recent years we have seen a slight uptick of deaths due to a conditioned called sickle cell trait (SCT).</strong> Not to be confused with sickle cell anemia (a disease formed by two abnormal sickle hemoglobin genes), SCT is genetically acquired, but will not result in a disease. In SCT a person may inherit one gene for sickle hemoglobin and one for normal hemoglobin. During extreme exertion, the sickle hemoglobin can change the shape of red blood cells from round to a quarter-moon “sickle” shape.</p>
<p><strong>Here is where <a href="https://www.ncbi.nlm.nih.gov/pubmed/15523207" target="_blank" rel="noopener" data-lasso-id="10853">SCT can be dangerous</a>: the sickle-shaped cells can “jam up” in the blood vessels, blocking blood flow to the muscles, kidneys, and other organs.</strong> This can lead to tissue death, organ failure, and rhabdomyolysis, the rapid breakdown of muscle cells that causes the release myoglobin and other cellular enzymes into the bloodstream. The harder and faster the person’s effort, the quicker and greater the chance sickling will occur.</p>
<p><strong><a href="https://www.omicsonline.org/scientific-reports/srep264.php" target="_blank" rel="noopener" data-lasso-id="10856">The U.S. Armed Forces</a> found that military recruits who possessed the SCT were thirty times more likely to die as compared to other recruits.</strong> They also found that those who have SCT are two hundred times more likely to experience exertional rhabdomyolysis. Most of the deaths occurred during basic training and fitness testing. They concluded the deaths were a result of intense conditioning that was well beyond the level the individuals were accustomed to.</p>
<p><strong>Regarding athletes, exertional sickling has <a href="https://breakingmuscle.com/junction-boys-syndrome-is-off-season-training-killing-football-players/" target="_blank" rel="noopener" data-lasso-id="10858">killed at least fifteen football players</a> in the past forty years.</strong> It has killed or nearly killed several college or high school basketball players while training (two were females), usually during court sprints, track laps, or a long training run (why it can also occur in distance racing). Two 12-year-old youth training for football have died in the past seven years alone from sickling. Of <a href="http://www.ccjm.org/content/72/3/176.full.pdf" target="_blank" rel="noopener" data-lasso-id="10859">136 sudden, non-traumatic sports deaths in high school</a> and college athletes over the past decade, seven (5%) were attributed to exertional sickling.</p>
<p><strong>Compounding the sickling can be exertion in the heat and/or high altitude, being dehydrated, and being asthmatic.</strong> A high percentage of sickling deaths have occurred in football players during the hot summer months of <a href="https://breakingmuscle.com/junction-boys-syndrome-is-off-season-training-killing-football-players/" target="_blank" rel="noopener" data-lasso-id="10861">off-season conditioning</a>. Regarding altitude, it was well publicized recently that Pittsburgh SteelersSafety <a href="http://www.huffingtonpost.com/2012/09/05/ryan-clark-pittsburgh-steelers-sickle-cell-campaign_n_1858822.html" data-lasso-id="10863">Ryan Clark</a> was not allowed to travel with the team to Denver due to his SCT.</p>

<p><strong>Who can get the sickle cell trait?</strong></p>
<p>The <a href="http://www.annualreviews.org/doi/abs/10.1146/annurev.genom.4.070802.110356?journalCode=genom" target="_blank" rel="noopener" data-lasso-id="10866">sickle cell gene originated</a> as a natural defense against the disease malaria in sub-Saharan Africa. <strong>Consequently, approximately 8% of African Americans have SCT. </strong>Additionally, those with a Mediterranean, Middle Eastern, Indian, Caribbean, and South and Central American ancestry are also at risk. Caucasians can also obtain it, but the odds range from 1 in 2,000 to 1 in 10,000.</p>
<p><strong>How is it diagnosed?</strong></p>
<p>All fifty states screen at birth. This provides a base marker. <strong>However, it is recommended a pre-participation physical examination be performed in all athletes via a blood test.</strong> If SCT is found, notification should be given to the parents, healthcare provider, and those responsible for determining the medical eligibility for participating in sports.</p>
<p><strong>How do you identify a sickling collapse?</strong></p>
<p>A sickling collapse can be incorrectly identified as either a cardiac event or heat collapse. Know that a cardiac event is usually instantaneous and heat collapse occurs after lengthy period of exertion.</p>
<p><strong>With a sickling collapse:</strong></p>
<ul>
<li>No muscle cramping occurs.</li>
<li>It occurs within the first half hour of activity.</li>
<li>Core temperature is not significantly elevated.</li>
<li>The pain of sickling is much less than heat-cramping pain.</li>
<li>Heat crampers hobble to a halt with “locked-up” muscles, while sicklers slump to the ground with weak muscles.</li>
<li>Heat crampers thrash and scream in pain with rock-hard contracted muscles. Sicklers lie calmer with muscles that look and feel normal.</li>
<li>Sicklers who are identified in the early stage can recover faster than those suffering from heat cramping.</li>
</ul>
<p>This is the norm, but sickling can also manifest differently. Some sicklers are stoic, and simply state they cannot continue the activity. Given ample rest time, the sickle red cells regain oxygen in the lungs and most then revert to a normal shape. The sickler then feels better and is able to them continue. This is a desirable situation.</p>
<p><strong>In the event of a sickling event, treat it as a medical emergency by doing the following:</strong></p>
<ul>
<li>Check vital signs (heart rate, blood pressure).</li>
<li>Administer high-flow oxygen (if available) at 15 liters per minute by using a non-rebreather face mask.</li>
<li>If necessary, cool the athlete.</li>
<li>If the athlete is mentally challenged or their vital signs regress, call EMS, use an AED, start an IV, and/or transport the person to a hospital.</li>
<li>Inform physicians to expect exertional rhabdomyolysis and the athlete will be in dire conditions when they arrive at the facility.</li>
</ul>
<p><strong><a href="https://www.ncbi.nlm.nih.gov/pubmed/15523207" target="_blank" rel="noopener" data-lasso-id="10868">Simple precautions can be used</a> so that no athlete is ever disqualified from training and competing.</strong> The following guidelines should be followed:</p>
<ul>
<li>Build up training slowly with paced progressions.</li>
<li>Allow longer periods of rest and recovery between bouts.</li>
<li>Make sure athletes participate in preseason strength and conditioning programs that enhance their abilities.</li>
<li>Performance testing should be sports-specific. Athletes with SCT should not be subjected to performance tests such as mile runs, repeat sprints, or anything far-removed from the demands of their sport.</li>
<li>When symptoms as muscle cramping, pain, swelling, weakness, tenderness, and the inability to catch one’s breath occur, cease the activity. Err on the side of caution.</li>
<li>It is proven that sickle-trait athletes who can set their own pace seem to do be safe.</li>
<li>Be vigilant with athletes who are susceptible to environmental heat stress, dehydration, asthma, any illness, and altitude predisposition. These can exacerbate the onset of sickling with intense physical exertion.</li>
<li>When training or competing in extreme heat, adjust work/rest intervals if possible.</li>
<li>Emphasize hydration!</li>
<li>If asthmatic, use an inhaler.</li>
<li>Educate athletes with the sickle cell trait to report any symptoms immediately such as fatigue, difficulty breathing, leg or low back pain, or leg or low back cramping.</li>
</ul>
<p>By using proper screening, taking the required precautions, and using sensible training plans, it will allow those with the sickle cell trait to train and compete without issue.</p>

<p>Sickle cell anemia is a genetic disease in which oxygen-bearing red blood cells form an abnormal sickle or crescent shape. The sickle-shaped cells are fragile, unlike healthy disc-shaped cells, and can get stuck easily in small blood vessels or even break into pieces.</p>
<p><strong>A <a href="https://www.ncbi.nlm.nih.gov/pubmedhealth/PMH0001554/]" target="_blank" rel="noopener" data-lasso-id="7066">recent study</a> confirms 25-year-old research that demonstrated a link between sickle cell trait and sudden cardiac death among young, athletic African-American males.</strong></p>
<p>Sickle cell anemia is a genetic disease in which oxygen-bearing red blood cells form an abnormal sickle or crescent shape. The sickle-shaped cells are fragile, unlike healthy disc-shaped cells, and can get stuck easily in small blood vessels or even break into pieces.</p>
<p>Consequently, this can alter healthy blood flow and delivery of oxygen. Sickle cell disease is much more common in people who are of African and Mediterranean descent. This trait is also seen in people from South America and Central America, the Caribbean, and the Middle East.<a href="http://www.eurekalert.org/pub_releases/2012-07/mhif-sct072312.php" target="_blank" rel="noopener" data-lasso-id="7067"><sup>1</sup></a></p>
<p><strong>For the past 23 years the <a href="https://mplsheart.org/" target="_blank" rel="noopener" data-lasso-id="7068">Minneapolis Heart Institute Foundation</a> has kept a registry of sudden death incidences in athletes. Of the 2,462 athletes listed, there were 23 who died of sudden cardiac death.</strong></p>
<p>Of those 23 athletes, 21 were male, and all were African Americans. Most of the <a href="https://breakingmuscle.com/junction-boys-syndrome-is-off-season-training-killing-football-players/" target="_blank" rel="noopener" data-lasso-id="7069">deaths occurred during football conditioning drills at a time early in the season</a>, and an in environment where athletes were exposed to high temperatures.</p>
<p>The research from the recent study concludes that the sickle cell trait can in fact be correlated with unpredictable sudden collapse death in African American male football players.<a href="http://www.eurekalert.org/pub_releases/2012-07/mhif-sct072312.php" target="_blank" rel="noopener" data-lasso-id="7070"><sup>2</sup></a></p>
<p>&#8220;The registry was initially started by Dr. Maron to help the medical community understand why any athlete would collapse on a field,&#8221; explained the study&#8217;s lead author Kevin M. Harris, MD, co-director of the Acute Aortic Dissection Program and director of the echocardiography laboratory at the Minneapolis Heart Institute at Abbott Northwestern Hospital in Minneapolis.</p>
<p>&#8220;We decided to assess the connection between the sickle cell trait and sudden death within our large registry,&#8221; Harris continued. <strong>&#8220;As a result, we have developed the first sizable series of competitive athletes in whom sickle cell trait was associated with otherwise unexplained sudden, unexpected collapse and death.&#8221;</strong><a href="http://www.eurekalert.org/pub_releases/2012-07/mhif-sct072312.php" target="_blank" rel="noopener" data-lasso-id="7071"><sup>3</sup></a></p>
<p>The results of this study may help those who may have the sickle cell trait be more cautious and aware of the risks involved. All African Americans are tested at birth, so they know early on whether they possess the trait.</p>
<p><strong>Since sickle cell anemia affects about eight-percent of African Americans, it is imperative everyone understands the risks involved and takes the necessary precautions to avoid tragedies such as sudden death on the playing field.</strong><a href="http://www.eurekalert.org/pub_releases/2012-07/mhif-sct072312.php" target="_blank" rel="noopener" data-lasso-id="7072"><sup>4</sup></a></p>
